Clearance Forms

All clearance forms are to be completed online through Sportsware.

Instructions on how to complete the forms can be accessed by following this link: Sportsware.

If you have an existing Sportsware account, you will need to contact the Sports Medicine Staff to create a new account.

Minors will need to print out their online pdf forms and have their parent/guardian sign. Please submit to the Sports Medicine office located in the southeast corner of Cook Gym (Room G-102).

When getting a physical through your own doctor, please print out and have this form filled out Physical Form.
